Financial Policy for Patients with Health Insurance:

This is a partial list of major insurance plans. Please check with your plan. We do except insurance assignment on most insurance plans In-Network and Out-of-Network.

For In-Network or Out-of-Network:

All co-payments, co-insurance, deductibles and any non-covered services are due at the time of service. We will submit a claim form one (1) time on the patient's behalf. We expect payment within 30 days from your insurance plan as required by Ohio law. You the patient are responsible for payment of all services your insurance company may deny, reject, or fail to pay. We except payment by check, cash, MasterCard, Visa, American Express and Discover credit cards.
